Hannah Rai, FNP-C

Hannah Rai, FNP-C, brings extensive experience in providing compassionate and comprehensive care to patients across diverse clinical settings. With 9 years of RN experience and board certification as a Family Nurse Practitioner, she has honed her skills in managing both acute and chronic conditions, from family medicine to pediatrics and women’s health.

Hannah has cared for patients of all ages, collaborating with interdisciplinary teams to deliver personalized care plans. Her previous roles include monitoring chronic conditions such as hypertension, diabetes, and heart failure, as well as diagnosing and treating orthopedic injuries with imaging guidance, such as X-rays, CT scans, and MRIs. She has also played a key role in women’s health, performing gynecological exams, counseling patients on pregnancy planning, and providing education on preventive care.

Throughout her career, Hannah has been committed to empowering patients through education, ensuring they understand their health conditions and treatment options. Her dedication to patient-centered care has fostered trust and positive outcomes across the communities she has served.
